Support!SummaryThis position will work as part of a team that
utilizes a Harm Reduction and Housing First Model to assist people
by meeting them where they are, with housing as a primary focus.
Team members will respond to each participant's unique needs and
goals by providing on-going support, advocacy and assistance in
accessing community services to ensure stability in housing and
integration in the community. Case Managers must be knowledgeable
about and able to respond effectively to the complexities of
participants' lives and the co-occurring challenges and conditions
associated with long-term homelessness.We serve primarily BIPOC
women, trans and non-binary people who have experienced
homelessness.Primary Responsibilities
- Provide Relevant Support to participants to Obtain and Maintain
Housing: includes outreach, networking, landlord relationships,
finding and securing stable housing.
- Provide Quality Case Management: maintain a case load of 15-20
participants, providing weekly support, crisis management,
assessment and goal planning.
- Complete Accurate Documentation.
- Active Team member: in a small, supportive team environment.Job
Requirements:
- Bachelor's degree in human services or equal experience in
field is required.
- Three or more years of experience working with single adults
who have experienced long-term homelessness or personal experience
with homelessness.
- Must understand and apply knowledge of domestic abuse,
childhood trauma, grief and loss, sexual violence and exploitation,
homelessness, social support systems, advocacy, tenant law and case
management in terms of providing services to women in crisis.
- Experience with mental health and chemical health issues.
- Must be computer proficient.
- Driver's license and car required.
- Home visitation requires the employee to be aware of personal
safety.Desired:
- Experience working with individuals who are homeless or having
personally experienced homelessness.
- Very broad knowledge of housing resources.
- Bilingual skills preferred.
